Practice Boundaries

Our catchment area for new patients covers the area for which we are the closest GP surgery.

We have a slightly wider catchment area for existing patients who move house but wish to remain registered at Hillfoot Surgery. This wider boundary has been set to reflect realistic travel times for GPs visiting the housebound.

Please ask at reception to see a map showing our precise boundaries.

To register please complete a GMS1 form (available to download via this link or you can pick one up from reception) and bring it to surgery along with a form of photo identification (e.g. passport or driving licence) and proof of address (e.g. recent utility bill).

Once we have your details on our computer system we will ask you to make an appointment with the Healthcare Assistant for a new patient check.
